Why join this webinar event?
Integrated care is more than a model of care. It’s a strategy for expanding access, improving outcomes, and prioritizing the needs, abilities, and preferences of patients.
In over four decades of work, the Colorado Coalition for the Homeless has worked to improve and advance integrated care at a 50,000 sq ft. Federally Qualified Health Center in Downtown Denver. But serving this population’s healthcare needs requires constant innovation, including how to meet clients in non-traditional spaces from the streets to transitional housing facilities, to the first moments of a housing intake.
Join data specialists and behavioral health experts from NextGen and Colorado Coalition for the Homeless as they share information on an innovative model and a real-world case study.
Attendees will learn how the Coalition captures critical clinical, social, and demographic information at intake to improve care coordination, identify benefits and health care insurance needs, and ensure individuals are connected to the right services despite funding constraints.
