Better Together—How One Organization Improves Patient Outcomes with Integrated Care

Jewish Family & Children’s Service (JFCS) has provided behavioral health and social services in their community for nearly 85 years. Four years ago, the organization introduced primary care into their service directory, and today—through a combination of office visits, home visits, and telehealth—they can provide up to 3,000 patient encounters per day.

The following session covers the positive impact JFCS has seen since integrating medical services into their behavioral health and social services organization.

What you'll learn:

  • A brief history of Jewish Family & Children’s Service
  • When and why they integrated primary care services
  • The integration transition process and lessons learned
  • Clinical outcomes and data
  • How technology impacts the ability to provide integrated care
